The Problem: Technology Without Operational Logic
Gartner predicts that more than 40 percent of all agentic AI projects will be discontinued by 2027. Industry data shows that the majority of enterprises have no defined AI operating model. The consequence is familiar: pilots succeed, scaling fails.
The pattern is always similar. A use case proves compelling in the pilot. The technology works. The result is impressive. And then what happens in every other AI project occurs — the transition to production fails. Not because of the technology. Because of the absence of operational logic.
What is missing is not a tool. What is missing is an operating model.
What an AI Operating Model Must Deliver
A governable AI operating model answers six operational baseline questions:
- Which AI systems are in production — and who knows?
- Which processes are they embedded in — with what allocation of tasks between human and system?
- Who is accountable — for the process, the system, the data, compliance?
- What happens when something goes wrong — escalation, fallback, incident response?
- Which data may the system use — and under what conditions?
- How is impact measured — and on what basis are decisions made?
Without answers to these questions, AI may be in use — but it is not governable. And ungovernable means: not production-ready.
The Framework: Eight Building Blocks
A robust AI operating model consists of eight structurally interdependent building blocks.
Building Block 1 — Strategic Frame AI initiatives without strategic alignment generate uncontrolled use-case proliferation without prioritisation. The strategic frame defines: What objectives does the organisation pursue with AI? Which use cases are strategically relevant? What is the risk appetite? Without this frame, no AI inventory exists — and without an inventory, no governance.
Building Block 2 — Process and Decision Architecture For every AI deployment in production, it must be defined: what does the system do, what remains with the human? Where does the process require human approval, where can the system decide autonomously? This task allocation is not optional — it is the operational foundation for human oversight and the direct requirement of the EU AI Act.
Building Block 3 — Roles and Responsibilities For every critical AI function, it must be clear: who is accountable, who decides, who reviews, who is informed. The RACI principle must be applied explicitly to AI roles. Without defined roles, there is no response capability when failures occur — governance violations are bypassed, not resolved.
Building Block 4 — Operational and Escalation Logic AI systems in production must know when they may act within their defined scope and when to hand over to a human. This means: defined confidence thresholds, limits on autonomous actions, mandatory approvals for sensitive output classes, fallback mechanisms, and a three-tier incident taxonomy. Escalation is not a failure mode — it is a normal component of controlled AI use.
Building Block 5 — Data and Knowledge Foundation Many AI systems fail not because of model quality, but because of an unclear or contradictory knowledge base. What must be governed: permissible data sources, currency assurance, access inheritance (least privilege), retrieval logic, and content stewardship. Without these rules, the knowledge foundation degrades — silently and without warning.
Building Block 6 — Architecture and Integration Governability requires a system architecture in which AI does not stand as a black box alongside the application landscape. Architecture-relevant decisions include: integration patterns, policy enforcement points, logging architecture, platform selection, and lifecycle management. Without technical enforcement, governance remains documentary.
Building Block 7 — Governance and Compliance Governance that exists only in documents is not governable. Approval pathways by risk class, technically enforced policies, structured documentation obligations (the EU AI Act requires complete technical documentation for high-risk systems), and defined change processes are not aspirational — they are operational requirements.
Building Block 8 — Monitoring and Performance Measurement Without systematic monitoring, there is no basis for governance. The monitoring system is structured across four levels: operational metrics (availability, error rate, cost), quality metrics (output acceptance rate, human correction rate, hallucination rate), impact metrics (process throughput, error reduction), and governance metrics (policy compliance rate, audit readiness, time-to-detect on incidents).
Six Common Anti-Patterns
Without a governable operating model, the same patterns recur in practice:
Tool-driven adoption: The organisation starts with a tool and searches for a sensible use case afterwards. The consequence: adoption without impact; usage drops to a low level after the initial phase.
Prompt tinkering: Knowledge and logic reside in individual prompts or chat histories. No versioning, no reproducible operational logic. Output quality depends on the competence of individuals — each personnel change introduces quality risk.
Shadow agents: Business units build solutions outside governance standards. Local efficiency, global risk: uncontrolled data access, missing audit trails, unclear accountability.
Compliance as afterthought: Regulatory requirements are addressed only after a system has been technically implemented. In the context of the EU AI Act — fully effective from August 2026 — this can result in substantial remediation costs or the complete shutdown of non-compliant systems.
Scaling without quality proof: A pilot with locally positive effects is transferred directly to a broader context. Quality issues that were compensated by individual attention in the pilot become visible at scale.
Unclear accountability: When failures occur, no one knows which responsibility level is affected. Delayed responses, incomplete root-cause analysis, recurring failures.
What the Operating Model Concretely Delivers
When the eight building blocks work together coherently, six operational impact areas emerge:
Manageability: The organisation can answer at any time — which AI systems are in production, in which processes, with which risk classification?
Accountability: When failures or regulatory audits occur, responsibility is assigned — documented, not implied.
Reproducibility: Identical inputs produce comparable outputs under identical conditions, regardless of who operates the system.
Scalability: New use cases build on existing architecture, governance, and operational building blocks. The effort for each additional use case decreases — marginal costs fall, governance quality remains constant.
Auditability: Decisions, approvals, and system behaviour are traceable in complete audit trails. An external review can be conducted without ad-hoc reconstruction.
Economic governability: On the basis of operational and impact data, the organisation can decide: which use cases deliver demonstrable value? Where are risks rising disproportionately? Evidence-based portfolio management instead of anecdotal success reports.
Maturity Model: Five Stages
Not every organisation needs to fully develop all eight building blocks immediately. The framework defines five maturity stages:
Stage 1 — Experimental: Local tools, individual prompts, no standards, no governance. Required action: inventory and initial prioritisation.
Stage 2 — Controlled in Pilot: First prioritised use cases with defined process linkage. Operational responsibility still open. Required action: define accountability before go-live.
Stage 3 — Stable in Production: Multiple use cases with clear process logic, defined escalation logic, and systematic monitoring. Required action: standardise architecture building blocks, build portfolio management.
Stage 4 — Scalable: Portfolio of use cases across multiple business units, reusable governance building blocks, technically enforced standards. Required action: integrate AI operations into enterprise management.
Stage 5 — Integrated: AI is part of regular operational management, with measurable value contribution at enterprise level and continuous governance review.
Architecture and Governance as a Unit
A governable AI operating model is neither a pure management question nor a pure technology question. It is an architecture question in the sense of TOGAF ADM: architecture connects business requirements with information systems and infrastructure within a consistent overall framework.
This means: the Business Architecture defines process logic, role model, and human oversight. The IS Architecture describes AI agents as a new application class with integration, logging, and lifecycle logic. The Infrastructure Architecture establishes platform decisions, security mechanisms, and the monitoring stack.
Governance must be operationally effective — not merely documented. Governance rules that exist only in manuals are not governable. They must be operationalised in policy enforcement points, access control systems, and audit trails.
Conclusion
AI is not project technology. It is operational infrastructure — and requires an operating model accordingly.
This does not mean developing all eight building blocks fully and immediately. It means starting at a realistic maturity stage and progressively building an organisation that genuinely governs its AI systems: transparently, accountably, auditably — and in an economically controllable way.
The transition from anecdotal pilots to evidence-based portfolio management is the real maturity leap. It does not require a better model. It requires an operating model.
